Management team by directing daily operations and ensuring
high-quality patient care coordination Oversee discharge planning
to promote safe, timely transitions and reduce readmissions Provide
a second-level review of levels-of-care and escalate unresolved
discrepancies to leadership Coordinate with insurance providers and
third-party payers to support utilization management Collaborate
with clinical staff and community resources to optimize patient
outcomes and control costs Ensure thorough, timely documentation
and compliance with organizational policies Support staff
development through input on evaluations, workload assignments, and
mentorship Qualifications Active Registered Nurse (RN) license
required Experience in case management, discharge planning, or
utilization management (leadership experience preferred) Strong
ability to communicate with insurance/third-party payers and
financial services Proven team leadership and staff coordination
skills Excellent documentation habits and attention to regulatory
and clinical detail Knowledge & Skills Deep understanding of case
management processes and discharge planning Familiarity with
utilization management and payer communication Skilled in patient
care coordination and resource optimization Ability to lead,
mentor, and evaluate clinical staff Competent in clinical
