Job Title: RN Patient Care Manager - HospiceWe are seeking a compassionate and experienced RN to join our team as a Patient Care Manager in Hospice. As a key member of our care team, you will be responsible for ensuring the delivery of safe and efficient patient care that meets the highest standards of quality and regulatory compliance.Key Responsibilities:

• Lead and motivate a team of healthcare professionals to provide exceptional patient care.
• Ensure all staff are oriented and supported to meet their job performance goals.
• Liaise between staff, patients, and the medical community to ensure seamless care coordination.Requirements:
  • Current RN licensure in the state of practice and at least 4+ years of full-time experience as an RN or equivalent.
  • Current CPR Certification.
